This form assists with the claim by providing details about the items you are wanting to claim for. It allows you to list the contents you are claiming for, the make and model of the individual items, as well as the approximate age and replacement costs of the item.
Please include your Proof of Purchase or Proof of Ownership along with this form. This can be in the form of receipts, invoices, bank statements showing the purchase, packaging or user manuals or photos of the item (can be either in the foreground, or background).
All this information assists us with settling your claim as we may need to contact our suppliers for the most appropriate replacement.
